Liability waiver

1. Acknowledgment of Risks. Ice hockey is a fast, physical sport involving inherent and other risks, including collisions with players, boards, glass, goals, pucks, sticks, skates, ice, and equipment; falls; checking or other body contact; equipment failure; unsafe ice or facility conditions; acts or omissions of participants, coaches, officials, spectators, facility personnel, or others; communicable illness; transportation incidents; and delayed or unavailable medical care. Injuries may include cuts, bruises, dental or eye injury, sprains, fractures, concussion or other head injury, spinal injury, paralysis, permanent disability, or death. I understand that protective equipment and rules reduce but do not eliminate these risks. 2. Voluntary Assumption of Risk. I knowingly and voluntarily accept all risks of participation, whether known or unknown, foreseeable or unforeseeable, and whether arising from ordinary negligence or otherwise, to the fullest extent permitted by applicable law. I confirm that participation is voluntary and that I may stop participating at any time. 3. Participant Fitness and Responsibilities. I represent that the participant is physically and medically able to participate, has disclosed relevant limitations to appropriate league personnel, and will not participate while impaired or when participation would be unsafe. The participant will follow applicable laws, facility rules, league rules, officials’ decisions, codes of conduct, and reasonable safety instructions; use properly fitted required equipment; and promptly report injuries, concussion symptoms, unsafe conditions, or damaged equipment. To the fullest extent permitted by applicable law, I release and agree not to sue MPHL, the legal organization identified above, participating teams, host facilities, sponsors, sanctioning bodies, and each of their respective owners, directors, officers, members, employees, coaches, volunteers, officials, agents, contractors, medical personnel, and representatives (collectively, the “Released Parties”) for claims, demands, causes of action, losses, or damages arising from or related to the Activities, including claims based on the ordinary negligence of a Released Party. This release does not apply to liability that cannot lawfully be released, such as liability for gross negligence, reckless or intentional misconduct where applicable law so provides. 5. Indemnification. To the fullest extent permitted by law, I agree to defend, indemnify, and hold harmless the Released Parties from third-party claims, liabilities, damages, judgments, costs, and reasonable attorneys’ fees arising from the participant’s acts or omissions, violation of rules, or damage caused by the participant, except to the extent caused by conduct that cannot lawfully be indemnified. 6. Medical Care and Emergency Authorization. If the participant is injured or becomes ill and I cannot be reached, I authorize league, team, facility, emergency, and medical personnel to provide or arrange reasonable first aid, emergency transportation, evaluation, and treatment. I understand that the Released Parties do not guarantee medical personnel or services and that I am responsible for medical and transportation costs not covered by insurance. This authorization does not require any person to provide treatment. 7. Concussion and Return to Play. I understand that a concussion may occur without loss of consciousness and can be serious. The participant will immediately stop play and report suspected concussion symptoms. The participant will not return to play until permitted under applicable law, league policy, and medical guidance. 8. Insurance and Personal Property. I understand that MPHL and the Released Parties may not provide accident, health, disability, or property insurance for the participant. I am responsible for maintaining appropriate insurance and for loss of or damage to personal property, except as otherwise required by law.
